The delivery of population and prevention-based diabetes care requires restructuring our acute-care focused health system. This panel presentation will showcase the work of community clinics that are in the process of making system changes. The framework for a chronic disease model will be presented, and then Ms. Norman will moderate a panel of representatives from four clinics, who will describe how they implemented system changes in their programs.
